This is well written, concise and extremely helpful review of the topic. I have read it cover-to-cover several times and gotten something new and useful each time. It serves as a regular reference for me when I am talking to students and other clinicians about errors. Anesthesia led the way in improving patient safety monitoring and reducing error in human medicine and I hope we lead the way in veterinary medicine as well. This book is a good first step in that direction.
